Please download, complete and submit Service Provider Application below.

The information package will include the following:

• Contractor Information Application
• Original W9 Form

We also need you to send in with your completed application:

• Certificate of Insurance naming "Energy Industries, LLC and its subsidiary companies" as an additionally insured.
• Copies of your Contractor Licenses
• Original Workers Compensation Certificate
